Dog Licensing
Per Chapter 7.06 of the Centralia Municipal Code, it is unlawful for any person except licensed dealers to keep any dog two months of age or older in the city without procuring a license for such dog.
Proof of current rabies vaccination is required for all dog licenses.
The license is good for a single calendar year (January 1 through December 31).
Licenses are subject to a $20 late fee, per dog, if renewed on or after February 1 each year.
Upon qualifying, low-income seniors or low-income disabled citizens can get a lifetime tag for a one-time fee of $5. Physical proof of low-income must be brought into the police department.

For your convenience purchasing your dog tags can be done online or in-person. If you choose to renew online, please forward the proof of rabies via:

Apply for your license online and pay with a credit or debit card.
License Application
If you choose not to pay your fees online, you can still fill in the form and deliver it with proof of rabies to the Centralia Police Department at 118 W Maple Street, Centralia WA 98531. Paper forms are also available at the police department. If you apply in person, you will be given your tag. If you apply online, your dog tag will be sent to your home address.
